Care Coordination Specialist

Job Description

• Full-time Care Coordination Specialist at Community Care Alliance's BH Link, a 24/7 Rhode Island triage facility serving adults experiencing mental health and/or substance use crises.
• Provides client reception, clinical-team support, referral coordination, paperwork assistance, safety checks, transportation coordination, and treatment-bay readiness.
• Maintains documentation and logs, engages in insurance authorization, monitors facility cameras, and provides clinical and administrative support as needed.
• Requires a minimum associate's degree in human services or equivalent experience; 1-3 years of human services or relevant experience is preferred.
• Requires compassion, rapport-building, verbal de-escalation, teamwork, effective communication, multitasking, prioritization, and ability to use electronic health records; 1st, 2nd, and 3rd shifts are available on a 3-4 day work week.
