Case Manager Social Worker

Job Description

• Provide counseling, intervention, and social work services to patients, families, and significant others coping with illness, diagnoses, treatment, abuse or neglect, adoption, hospice, and end-of-life issues.
• Coordinate complex discharge planning and placement with patients, families, physicians, nurses, RN Case Managers, healthcare teams, outside agencies, ECFs, rehabilitation facilities, domestic violence agencies, and shelters.
• Assess healthcare needs, participate in multidisciplinary rounds and daily team meetings, identify referrals, convene family meetings, and document communication in the patient electronic database.
• Meet legal, regulatory, safety, HIPPA, quality-management, disaster-response, and continuing-education requirements, including reporting suspected child or adult abuse to the appropriate protective services agencies.
• Full-time, day-shift, on-site role in Livonia, MI, with Sunday and holiday on-call coverage; salary not disclosed.
