Job Description

• CHRISTUS Health is hiring a full-time Post Acute Care Coordinator supporting Population Health Care Management and value-based care ACO arrangements; location is listed as United States and work mode is not specified.
• Coordinate post-acute care transitions, facility outreach, expected discharge-date reviews, confirmed discharge notifications, and multidisciplinary communication.
• Process PHI, Plans of Care, clinical documentation, and medical records through the EMR; maintain the EPIC Post-Acute outreach report and accurate records.
• Required: 2 years of healthcare-setting experience, experience with EPIC and other EHR systems, and practical Microsoft Office experience; 2 years working with post-acute populations is preferred.
• Associate or bachelor's degree preferred; no licenses, registrations, or certifications required.
