Job Description
• Direct social service functions and develop programs addressing the emotional and psychological needs of residents across multiple levels of care and their families.
• Conduct admission social assessments, identify psychosocial needs, collect resident data, and maintain compliant health-record and care-plan documentation.
• Participate in interdisciplinary care planning, resident assessments, inter-facility transfers, admissions, discharges, and departmental communication.
• Advocate for residents and families, protect resident rights, provide family liaison and support meetings, and explain government and facility policies.
• Requires a State Social Worker license, a Bachelor’s degree in Social Work, four years of medically related/long-term care experience, and two years in a social-work supervisory capacity.
• Full-time position in Evanston, IL; work mode is not specified. Compensation is $67,295.00–$93,610.25 per year.
