Outpatient Case Manager

Job Description

• Outpatient Case Manager with Santa Fe Recovery Center, Inc. in Santa Fe, NM; full-time position with work mode not specified.
• Assess clients, triage them to the appropriate level of case management, and develop, monitor, and support Individual Service Plans.
• Connect clients with mental health, housing, employment, medical, financial, educational, childcare, and other community resources while coordinating with families, providers, and clinical teams.
• Provide person-centered support, independent-living and daily-skills training, follow-up, crisis intervention, appointment guidance, and continuity-of-care coordination.
• Maintain timely DAP notes, case files, confidentiality, and documentation in compliance with HIPAA and applicable local, state, and federal laws.
• Requires High School/GED plus CCSS certification, CPSW certification, or 4 years of Substance Misuse or Mental Health experience with the ability to obtain CCSS and/or CPSW; unrestricted NM Driver’s License and current CPR/First Aid certification or willingness to become certified.
