Job Description
• Coordinate, schedule, and assign palliative patients to providers while partnering with field staff and leaders to support patients.
• Conduct follow-up calls with patients and families to collect treatment-response data and serve as an informational resource for care teams, caregivers, payers, and community resources.
• Manage referrals to appropriate agencies and help clients achieve Care Plan goals and objectives.
• Assist with health-care-system issues and financial or social barriers, including interpreter, transportation, and prescription-assistance requests.
• Requires 2+ years in palliative, hospice, or home care; a health-related bachelor’s degree, medical records experience, and HCHB experience are preferred; electronic health record data-analysis experience is required.
