Job Description
• Provide, coordinate, plan, and document hospice nursing care for terminally ill patients from admission through discharge.
• Develop and implement individualized hospice care plans using physicians' orders and professional nursing skills.
• Ensure care complies with accrediting organization standards, federal and state regulations, reimbursement guidelines, and organizational policies.
• Full-time, 36-hour dayshift position in O'Fallon, IL, with on-call duty once per week and every fifth weekend; requires travel for home visits.
• Requires an Illinois RN license, BLS certification, valid driver's license, automobile insurance, reliable personal transportation, and willingness to travel within the service area.
