Job Description
• Provide social work interventions, discharge planning, patient advocacy, and care transitions for patients and families at St. Mary's Hospital in Richmond, VA.
• Identify patient needs using biopsychosocial, functional, cultural, spiritual, and financial factors; coordinate with patients, caregivers, and multidisciplinary healthcare teams.
• Complete timely patient-record documentation and incorporate patient and caregiver treatment goals and preferences into transition-of-care plans.
• Apply standardized practices for Advance Care Planning, Length of Stay management, readmission prevention, medical-necessity denial prevention, and Interdisciplinary Discharge Rounds.
• Required: Bachelor of Social Work, BLS from the American Heart Association, Social Worker licensure in the state of practice, and 1 year of clinical-setting experience; ACM or CCM certification and additional acute-care experience are preferred.
