Social Worker - Full Time - days

Job Description

• Provide psychosocial evaluations and interventions for outpatient patients ranging from infants to geriatrics.
• Assess patients' social, emotional, medical, nursing, home, financial, and community-resource needs.
• Develop and facilitate discharge and treatment plans in collaboration with interdisciplinary healthcare teams and the RN Case Manager.
• Maintain timely daily medical-record documentation containing all required regulatory components.
• Provide mental health evaluations, cognitive behavioral therapy, individualized/group therapy, and financial-resource assistance.
• Requires a Master’s in Social Work, New York State Medical Social Work license, and successful completion of a 6-month practice-based orientation; two years of postgraduate healthcare Social Worker experience is strongly preferred.
