Job Description

• Full-time outpatient dialysis Social Worker serving patients and families across DaVita’s Windham and Norwich locations, 32 hours per week.
• Support and advocate for patients transitioning to dialysis and managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.
• Help patients understand their rights and responsibilities while building meaningful, long-term relationships with patients and families.
• Requires an MSW specializing in clinical practice, compliance with state dialysis-practice regulations, and either an LCSW or LMSW pursuing LCSW; LCSW is preferred.
• Requires knowledge of government and private insurance programs plus proficiency in Microsoft Office and DaVita applications within 60 days; includes a $10,000 retention bonus.
