Social Worker - MSW (Inpatient)

Job Description

• Provide inpatient social work support and comprehensive services to patients and families within the Johnston Health healthcare team.
• Conduct mental, emotional, social, psychosocial, risk, and diagnostic assessments; develop and monitor individualized patient-centered care plans.
• Connect patients and families with community resources, mental health services, support groups, psychiatric evaluations, medication management, referrals, and other programs.
• Coordinate care across providers and settings, conduct follow-up assessments, communicate with payers for authorizations and reimbursement, and advocate for patient access and continuity of care.
• Requires a Master's Degree in Social Work; two years of healthcare experience is strongly preferred. Onsite, full-time day position in Smithfield, NC; $33.37–$47.97/hour.
