Job Description

• Supervises daily comprehensive care management activities and assigned social work staff, including hiring, performance evaluation, education, staff development, work allocation, and problem resolution.
• Assesses and coordinates patient care across case management, social work, utilization review, care coordination, care planning, delivery, and discharge planning.
• Develops multidisciplinary processes involving physicians, pharmacists, nurses, social workers, and other health professionals to improve clinical, financial, quality, and patient satisfaction outcomes.
• Implements standards of practice, standardized policies, and operational processes while ensuring compliance with institutional policies, regulatory requirements, ethical standards, and staff scope of practice.
• Requires a master's degree in Social Work, 3 years of experience, and one of the following licenses: LCSW, LMSW, or LMSW-AP.
• Full-time, location-based role in Round Rock, TX, scheduled for Saturday through Tuesday on 10-hour shifts.
